General Questions About SD Premier Clinics

We offer comprehensive cardiology and pulmonary services, including diagnostic testing (EKG, echocardiogram, stress test, Holter monitor, pulmonary function testing), interventional cardiology (angiogram, angioplasty, stenting), sleep apnea evaluation, and management of chronic conditions like hypertension, AFib, heart failure, COPD, and asthma.

Yes. We have board-certified cardiologists and pulmonologists in the same practice. This integrated model allows us to manage conditions that affect both the heart and lungs - such as sleep apnea, which impacts cardiovascular health - in a coordinated way.

Our clinic is at 655 Euclid Ave, Ste 301A, National City, CA 91950. We serve patients from across San Diego County, including South San Diego, Chula Vista, Bonita, and Coronado.

Appointments & New Patient Information

In most cases, no. Many insurance plans allow direct specialist access. If your plan requires a referral, your primary care provider can send one to us. Our staff can also help you navigate this.

New patients are typically seen within one to two weeks. For urgent concerns, please call us and we will do our best to accommodate.

Bring your photo ID, insurance card, a list of current medications (including dosages), any recent lab results or imaging, and prior medical records from previous cardiologists or pulmonologists. Arrive 10-15 minutes early to complete any remaining paperwork.

New patient consultations usually take 45-60 minutes. Your provider will review your history, discuss concerns, and outline next steps or any recommended testing.

Yes. Because our cardiologists and pulmonologists are in the same practice, we can often coordinate same-day appointments for patients who need both evaluations.

Insurance & Billing Questions

Yes. We are a Medicare-participating provider. Medicare covers medically necessary services such as office visits, diagnostic testing, and procedures.

We accept self-pay patients. Please contact our office to discuss payment options.

You can pay by phone, mail, or in person. Once our patient portal is live (Spring 2026), you will be able to pay online through the portal.

Most insurance plans cover these tests when medically necessary. Our team verifies coverage before your appointment.

Questions About Your Medical Care

If you are experiencing chest pain, severe shortness of breath, sudden weakness, or any other life-threatening symptoms, call 911 immediately. Do not wait for a clinic appointment.

Call our office at (619) 472-4900. Please allow 48-72 hours for refill requests. Once the patient portal is active, you can also request refills online.

Please call us at least 24 hours in advance. This allows us to offer the time to another patient.

For some tests (e.g., stress test, pulmonary function test), we may ask you to temporarily hold certain medications. You will receive specific instructions when you schedule. Do not stop any medication without confirming with our office.

You can sign a release of information form (available in our office or on our Forms page) to authorize transfer. We can also request records from your previous provider on your behalf.
