Our Responsibilities
We are required by law to maintain the privacy of your PHI, to provide you with this notice, and to follow the terms of this Notice.
SD Premier Clinics (the Practice) participates in an Organized Health Care Arrangement (OHCA) with the University of California, San Diego Health System (UCSD) for purposes of compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). This notice is jointly used by and jointly describes the practices of all participants within the OHCA, including, without limitation any health care professional authorized to enter information into your medical record. Your health information is integrated into UCSD's electronic health record keeping system. UCSD also has its own Notice of Privacy Practices that can be accessed at http://health.ucsd.edu/hipaa/Pages/hipaa.aspx.
The OHCA will follow the terms of this joint notice. The OHCA may share medical information with each other for treatment, payment, or health care operations related to the OHCA as well as for research related purposes conducted at UCSD and at all related UC Medical Groups and UC Hospitals.
Information We Collect
We may collect personal and medical information, including name, phone number, email address, date of birth, insurance details, information provided when booking appointments or contacting us, medical and health information necessary for your care.
When you visit our website, we may also collect IP address, browser and device information and pages visited and time spent on the site.
How We Use and Disclose Your Information
Treatment: We may use and disclose your PHI to provide, coordinate, or manage your health care and related services among health care providers.
Payment: We may use and disclose your PHI to bill and collect payment for services provided to you.
Health Care Operations: We may use and disclose your PHI for clinic operations such as quality assessment, training, licensing, audits, and administrative activities.
Other Uses: We may disclose your PHI without authorization as required by law or permitted under HIPAA, including public health, health oversight, judicial, and law enforcement purposes.
Uses and Disclosures Requiring Authorization
We will not use or disclose your PHI for marketing purposes, sale of information, or most uses of psychotherapy notes without your written authorization.

We may update this Privacy Policy and Notice periodically. Updates will be posted on this page with a revised effective date.
